Hydration signals are useful precisely because they are simple. They become misleading when one clue is treated as a diagnosis.

Thirst is useful, but context changes it

  5. Medical boundaries: fluid restrictions, pregnancy, kidney or heart disease, and relevant medications override generic rules.

Do not use symptoms to guess between emergencies

Headache, nausea, and fatigue can occur with dehydration, overhydration, or heat illness. Confusion, seizure, collapse, inability to drink, or severe worsening symptoms require urgent assessment. Do not assume the answer is simply more water.

Best use: Treat thirst and urine color as low-cost prompts. Combine them with context and trends; escalate unusual colors or severe symptoms instead of self-diagnosing.
