Extended fasting and OMAD explained
Compressing all your food into one meal or going days without eating is a different proposition than a 16-hour overnight fast — the evidence base is thinner, and the risks are real.
Where this differs from standard intermittent fasting
Common intermittent fasting protocols (16:8, for instance) mostly compress eating into a shorter window without necessarily reducing total food eaten. OMAD (one meal a day) and extended fasting (24+ hours without food) go further — and the evidence quality drops accordingly, with far fewer controlled human trials than for standard time-restricted eating.
What OMAD tends to do
- Makes hitting protein and micronutrient targets harder — fitting 100g+ of protein and a full day's micronutrients into one sitting is a real practical challenge
- Often reduces total calories — simply through the difficulty of eating a full day's calories in one meal, which is frequently the actual mechanism behind any weight loss, not something metabolically special about meal frequency
- Can affect blood sugar regulation — a very large single meal produces a bigger glucose and insulin response than the same calories spread across the day
Extended fasting (24+ hours)
Multi-day fasting shows some evidence for short-term metabolic markers in small studies, but longer fasts carry real risks: electrolyte imbalances, gallstones (from prolonged periods without gallbladder stimulation), and — critically — muscle loss, since the body draws on protein for gluconeogenesis once glycogen is depleted.[1]
Who should avoid this entirely
People who are pregnant or breastfeeding, have a history of disordered eating, are underweight, have diabetes on medication, or have any condition requiring regular food intake should not attempt extended fasting without direct medical supervision.[1]
