Does creatine cause hair loss?

The concern grew from one small study that measured a hormone—not hair. In 2025, a trial finally measured both.

The short answer

Current evidence does not show that creatine monohydrate causes hair loss. The first randomized trial to measure hair density, follicular units and hair thickness found no difference between 5 grams of creatine per day and placebo over 12 weeks. That is reassuring, but it is one modest study in resistance-trained men—not proof about every person, dose or multi-year use.

Why people connected creatine to hair loss

Androgenetic alopecia—common pattern hair loss—is influenced by genetics and androgen signaling, including dihydrotestosterone (DHT) activity in susceptible hair follicles. If a supplement reliably raised DHT, a concern about hair could be biologically plausible. But plausible is not the same as demonstrated.

The creatine rumor traces mainly to a 2009 crossover study of 20 college-aged male rugby players. Participants completed a seven-day creatine loading phase at 25 grams per day followed by 14 days at 5 grams per day. Serum testosterone did not change, while DHT rose from baseline and the DHT-to-testosterone ratio changed.

That study measured blood hormones for three weeks. It did not count shed hairs, examine follicle miniaturization, measure hair density or diagnose hair loss. Its result created a hypothesis worth testing; it did not establish that creatine made anyone bald.

Why the 2009 result was not enough

A 2021 expert review summarized the literature and concluded that the evidence at that time did not indicate that creatine increased testosterone, DHT or hair loss (Antonio et al.). Still, the obvious gap remained: nobody had directly studied hair.

What the 2025 trial measured

The newer randomized, double-blind trial assigned resistance-trained males aged 18–40 to 5 grams per day of creatine monohydrate or a maltodextrin placebo for 12 weeks. Forty-five were recruited and 38 completed the study. Researchers measured total testosterone, free testosterone and DHT. They also used a trichogram and FotoFinder imaging to assess hair density, follicular-unit count and cumulative hair thickness.

The study found no significant group-by-time differences in the hormone measures or hair outcomes. In plain language, the creatine group did not show worse hair density, fewer follicular units or thinner cumulative hair than placebo over the study period. What improved

This was not another study inferring hair effects from a blood marker. It directly measured several features of hair and follicles while using a placebo-controlled design.

What the trial still cannot prove

One 12-week study cannot establish that no creatine user could ever notice shedding. Pattern hair loss unfolds over longer periods and is strongly influenced by genetics, age and baseline follicle sensitivity. The study was limited to men aged 18–40 who resistance trained. It did not focus only on people with a strong family history or active androgenetic alopecia, and it did not test a loading phase.

The fair conclusion is stronger than “we know nothing” but narrower than “impossible.” The best direct trial found no adverse hair signal over 12 weeks at 5 grams per day. Longer and larger studies in more varied populations would increase certainty.

Anecdotes deserve care, not automatic dismissal

People sometimes report shedding after starting creatine. Their experience is real, but the timing alone cannot identify the cause. Hair shedding can reflect androgenetic alopecia, telogen effluvium after illness or stress, weight loss, low energy intake, iron or thyroid problems, medications, scalp conditions and normal fluctuation. Starting a supplement may also make someone inspect their hair more closely.

Stopping and restarting a supplement is not a controlled experiment when training, diet, stress and hair cycles change at the same time. If shedding is sudden, patchy, painful, accompanied by scalp changes or persistent, a clinician or dermatologist can investigate causes that should not be missed.

What creatine actually does

Creatine helps replenish phosphocreatine, which supports rapid energy production during repeated high-intensity efforts. It is one of the most studied sports supplements. The NIH Office of Dietary Supplements notes that creatine can improve strength and power for repeated short, intense activity and that creatine monohydrate is the most widely studied form (consumer fact sheet).

Common practice uses 3–5 grams per day. A loading phase can saturate muscle sooner but is not required. Initial weight gain often reflects more water stored with creatine in muscle, not rapid fat gain.

If you are worried about your hair

  1. Do not use a supplement you do not want. Creatine can help training performance, but it is optional.
  2. Keep the question specific. Are you seeing more hairs in the shower, visible thinning, a changing hairline or a patch? Those are not identical patterns.
  3. Take consistent photos. Same light, angle, hairstyle and interval are more useful than daily mirror checks.
  4. Review other changes. Illness, calorie restriction, rapid weight loss, stress and medication changes can matter.
  5. Get qualified help for ongoing loss. A dermatologist can distinguish types of hair loss and discuss treatments. Do not start or change prescription hair-loss medication based on a supplement article.

Sources and limitations

Several authors in creatine research have professional connections to supplement science or industry; study design, replication and direct outcomes matter more than authority alone. This article is educational and does not diagnose hair loss.

Bottom line

The creatine–hair-loss claim began with an indirect, small study. The first trial to directly examine hair found no harm after 12 weeks at 5 grams per day. That makes the claim less credible, not mathematically impossible. If creatine helps your training and you want to use it, current evidence does not provide a strong reason to expect hair loss.
